PROCEDURES
The Healthcare commission meets at 13.30 on Tuesday, Thursday and Friday, 3 days a week.
DISABLED REPORTS
For the Disability Report of our patients' applications are received. EBYS record of the petition or cover letter is taken. Disabled (Home Care Fee, to benefit from the rights of the disabled, to submit to the Court, Special Consumption Tax (PAID) etc.) are directed to the relevant sections.
It would be beneficial for our patients who will receive a Disabled Health Board Report to learn from (0374)2753030-2569 (Health Board extension) that the committee members are complete before they bring their patients.
Your Disability Report results are only given to you or your first-degree relatives in return for your identification and signature.
HEALTH STATUS REPORT
TAF (Turkish Armed Forces), License to Carry or Possess Weapons, Being a Private Security Officer [Armed or Unarmed], Starting a Work, School Registration, Reports based on appointment, Citizenship, Educational Nursing Home, etc. Our patients who apply for the disease are directed to the departments related to their diseases.
At the last stage, it is checked whether the examinations have been completed completely, and reports are written and prepared for the board. It is delivered to the patient at the latest at 16.30 on the same day. In addition, Official applications with the cover letter are sent to the institution from which the cover letter is received, with a reply.
